Document Glossary
Form 1099-MISC
Please provide a copy of Form(s) 1099-MISC, from most recent tax year.
Form W2
Please provide copies of all of your Form W2 for the previous year. This is provided to you by your employer. Alternatively, you may provide a year-end pay stub instead of a Form W2.
Government Issued Photo ID
Please provide a legible copy of a valid government issued photo ID such as Driver's License, State ID Card, Permanent Resident Card ("Green Card"), Official Name Change Document or US Passport.
IRS Form 4506-T
Form 4506-T authorizes Lending Club to request a transcript of your tax return from the IRS. Please click here to download and print this form. Be sure to sign the Form prior to submitting it to Lending Club. There is no charge for this service.
Please provide the following information on Form 4506-T:
Line 1a: Your full name
Line 1b: Your social security number
Section 3: Enter the address you used on your prior year's tax return (If you have recently moved, please be sure to use the address you provided when filing your most recent tax return.)
Section 6: Enter '1040' on the line for Transcript Requested & check Box C(Return Transcript)
Section 9: Tax Year: Please leave this section blank. ***This section will be completed by Lending Club ****
Military Leave and Earnings Statement
Please provide a copy of your two most recent Leave and Earnings Statements
Most Recent Bank Statement
Please provide all of the pages from the most recent monthly bank statement where your income is deposited.
Most Recent Federal Tax Return
Please provide a signed copy of the first two pages of your most recent federal tax return (Form 1040, 1040A or 1040EZ). Please also include Schedule C and/or Schedule E if you filed them with the IRS.
If you earned business income from a partnership, S-corporation or Limited Liability Company (LLC) please also include your Schedule K-1.
Proof of Alimony/Child Support
Please provide a copy of one of the following which indicates payment of alimony or child support: Divorce decree, separation agreement, legal agreement, or court decree.
Proof of Disability Insurance or Workers Compensation Benefits
For long term disability, if applicable, please provide each of the following:
- 1. A copy of your most recent benefits statement or the award letter you received from your benefit provider, which is most likely your disability insurance company or your employer.
- 2. A copy of your completed claim form signed by your employer along with a letter from the insurance company showing your compensation status and the monthly benefits amount.
- 3. A copy of your social security award letter or Form SSA-1099.
For short term disability please provide each of the following:
- 1. A copy of your most recent benefits statement or the award letter you received from your benefit provider, which is most likely your disability insurance company or your employer.
- 2. A copy of the two most recent paystubs you received prior to becoming disabled.
- 3. A letter from your doctor confirming the date that you will be returning to work.
Proof of Pension or Annuity Income
If you receive income from a pension or annuity, please provide a copy of your award letter or your most recent statement. Alternatively, you may submit copy of Form(s) 1099-R for the previous tax year.
Proof of Social Security
Please provide a copy of your Social Security Award Letter or Form SSA-1099 for the previous tax year. Alternatively you may provide copies of the three most recent bank statements (all pages) for the bank account where your Social Security payments are made via direct deposited.
If you can't locate your most recent social security letter you can find it at the Social Security Administration website.
Recent Credit Card and/or Loan Statement
Please provide a recent copy of the credit card and/or loan statements that show the balance you are seeking repay with the proceeds of this loan. The statement(s) must show your current balance, your name, and your current mailing address.
Recent Home Utility Bill
Please provide a copy of your most recent utility bill for your current residence, which has your name on the document. Acceptable utility bills may be from your local gas, electricity, cable, waste management, automobile or homeowners/renters insurance or land line telephone companies. Alternatively, you may submit a valid government issued photo ID AND a recent credit card statement or pay stub instead of a utility bill provided that the document you submit shows your name and current residential address (no P.O. boxes).
Three Most Recent Bank Statements
Please provide all of the pages from the three most recent monthly bank statements where your income is deposited.
Two Most Recent Pay Stubs
Please provide a copy of your two most recent official pay stubs or payroll statements from your employer. The statements must show your year-to-date earnings.
Two Most Recent Federal Tax Return
Please provide a signed copy of the first two pages of your two most recent federal tax return (Form 1040, 1040A or 1040EZ). Please also include Schedule C and/or Schedule E if you filed them with the IRS.
If you earned business income from a partnership, S-corporation or Limited Liability Company (LLC) please also include your Schedule K-1.
Verification of "Doing Business As" Name
Please provide verification of the Doing Business As (D/B/A) name that you use to conduct business. Acceptable verification forms include registration with your secretary of state, your business license, or your professional certification.
Veteran Benefits Administration Benefits Letter
Please provide a copy of your Veteran Benefits Administration benefits award letter or all pages of your three most recent bank statements where your benefits are made via direct deposit. If you can't locate your most recent veteran benefits letter you can find it at here.

Line 1a: Your business legal name, as it appears on your tax returns. (If you are a sole proprietor, this is your own name.)
Line 1b: Your Taxpayer Identification Number, as it appears on your tax returns.
Line 3: Enter the address you used on your prior year's tax return (If you have recently moved, please be sure to use the address you provided when filing your most recent tax return.)
Line 5: 3rd Party Address: Please leave this section blank. ***This section will be completed by Lending Club***
Line 6: Enter the type of tax return the business filed (1040, 1065, 1120, or 1120s), and check box 6c.
Line 9: Tax Year: Please leave this section blank. ***This section will be completed by Lending Club***
Bank Statements for All Business Bank Accounts
Please provide the last 3 months of bank statements for the primary operating account(s) used by the business. This is the account you use to deposit your revenues and pay expenses. If the business regularly uses multiple accounts to deposit revenue and pay expenses, please provide statements for each of these accounts.
Business Tax Returns
Please provide a copy of the most recent tax returns the business filed. Please also include all schedules filed with the IRS. If you are a sole proprietor, this is your Form 1040 with Schedule C. For other types of businesses, this is your Form 1120, Form 1120S, or Form 1065.
